New Hire forms:
The following is a listing of the forms and documents the District must have on file for a new employee. It is arranged by employee classification as there are different requirements for each group of employees. Forms are available on this site; applications must be completed online in Applitrack (except volunteers). If you did not upload the required documents in Applitrack you must bring them to the Business Office. At some point, we must physically view the documents used to substantiate citizenship on the I-9. Plus, we must have a copy of your current social security card in your HR file. Submit all completed forms and/or documents to the Business Manager at the District Offices located in the Junior-Senior High School, 5876 County Road P43, Fort Calhoun, NE 68023. For the Child/Adult Abuse and Neglect Registry check, you will receive an e-mail with a link to the Nebraska Department of Health and Human Services website online application. Please complete at your earliest opportunity, you will be able to access the report as will the District. Please note the offer of employment is not final until the District has the background check report and the Child/Adult Abuse and Neglect Registry check report and deemed them to be clear.

- The W-4 Form

The W-4N – As of January 1, 2022, you must complete a form W-4N for Nebraska withholding allowances when you do a W-4 federal form. The minimum required withholding amount for the State of Nebraska is 1.5% of taxable wages. You are allowed to go below this minimum requirement, but if you do, you must substantiate the allowances you are claiming on form W-4N by completing the Special Withholding Procedures form below.
**The District only pays via direct deposit and provides pay stubs via the Employee Access Center with an e-mail notification. Therefore a bank account and e-mail address are requirements for employment.
***Please note: We must actually see and verify the documents you use to substantiate your right to work in the USA when completing the I-9 form.
Certified and/or Administrative employees:

Transcript(s) supporting degree and/or additional hours.
